16'' Wheel to 18"???

I have a 16" drum brake set up rim on the rear of my bike right now and I bought a 18" rim the same style except it is disk brake. The 18" rim I bought came with a sprocket will the chain that is on the bike now fit still if I just swap out the rims and sprocket?

I would recommend doing some searches. Lost has been written about the switch to an 18" rear wheel (stock from '77-'79) and the difference in the number of teeth between the two rear sprockets.
