I'd recommend you use a smart phone to track your speed. There are several free app's that will record your speed and locations. Just find a straight road, hit it on, and go up and down it at the same speed. compare it against the speedo.
You might want to check the speedo sensor on the front...
Well that might be bad (transmission is going), or it could just be a personal thing. (Your way of shifting. Not a romance thing or whatever)
Does it apply to both up and down shifts? Does it ever work? This is while moving correct?
Have you checked oil levels? This has to be done with the...
Can you clarify what you mean by 'loose'? Also 'changing gears'? Do you mean the foot shifter feels loose and you can't get into gear? Have you tried shifting hard into it and seeing what happens? (Not like jumping onto it, just a bit of force)
Something that is overlooked:
Bad petcocks. It can cause a ton of weird issues. My bike had high RPM issues and would sometimes hesitate at or near WOT. I replaced my petcock and its all good now.
It should have 5. The manual you are reading is probably for the SECA. Which has the same frame, suspension, engine, but different transmission and some body parts.
Get a 16t up front. It helps a lot.
If you are bored, got money, and want 6 speeds you can swap in the seca transmission...
I'm not 100% sure if the DOHC is suppose to pop either, the mechanism could just be frayed and old on mine. I know it did pop off and it hasn't presented an issue yet. 1000 miles or so.

You can totally fit a DOHC engine into an xs400 SOHC frame! ..you do have an engineering degree, a welder, tube bender, and a good insurance policy right??
Missing o rings on a carb is always ba.d I'd replace that asap.
It should be washer -> oring to squish that oring in.
Good on you for keeping at it though. Make sure you have a filter for the fuel because these things can be VERY touchy to particles.
